NEILS is dedicated to improving the lives of the disabled and aging in our community and helping them be as independent as possible. Many disabled or aging people have to go to a nursing home for long-term rehabilitation and find it difficult transitioning back to their home, possibly because they have new physical challenges, or because they don’t have a home to go back to. Or, a disabled child matures to adulthood and wants to live independently, but needs help getting a home of their own.

 

NEILS can help with their Nursing Home Transition Program – if you or a loved one wants to transition from the nursing home back to independent living or from your parent’s home to a home of your own, contact us. We can help you find housing, and get you the support you need to live on your own. Contact us to schedule Options Counseling with us, to start the process of discovering if you are able to live independently, and if so, get you into a home. Together, we will work out what you will need and help you have a safe and healthy transition.

  • Sourcing financial assistance
  • Sourcing assistive technology
  • Providing home care services

 

You are not alone, we are here to help you in any way we can. It is possible to leave the nursing home, or your parent’s home if your health allows you to, and live as independently as you are able. We are here to fill in the gaps where you will not be able to be so independent. Together, we can do this!
